A Distributed Healthcare Story

Distributed healthcare is a model of distributed connected care touch-points that extends care beyond the hospital - integrating into our daily environments via a myriad of convenience access points. Patients receive care as a seamless journey of interoperable systems that exists in our homes, our communities and even in vehicles on the move. These care access points are linked via digital platforms, where population health is holistically managed via digital command centers that manage care delivery and also predictively anticipate loading and demand.
